Management 0f Fingertip Amputations: Contemporary Surgical Options

ResumenFingertip amputations are among the most common injuries of the hand and may result in substantial functional, sensory, aesthetic, and occupational impairment. Treatment options range from healing by secondary intention and semi-occlusive dressings to revision amputation, skin grafting, local or regional flaps, composite grafting, microsurgical free tissue transfer, and distal replantation. No single technique is appropriate for all injuries. Management should be individualized according to the level and geometry of tissue loss, mechanism of injury, bone or tendon exposure, condition of the nail complex, patient age, occupation, comorbidities, expectations, and available microsurgical resources. Current evidence increasingly supports conservative management for selected injuries because semi-occlusive dressings can achieve satisfactory pulp regeneration, sensibility, cosmesis, and patient-reported outcomes with low complication rates. Surgical reconstruction remains essential for larger defects, exposed critical structures, significant loss of length, unstable nail support, or cases in which durable and rapid coverage is required. This review summarizes contemporary management principles for fingertip amputations, compares the principal reconstructive options, and proposes a practical treatment algorithm focused on preservation of length, sensation, nail function, and early return to activity.es_ES
